The seminar explored what happened and what has happened since in Norfolk. 

During April 2019, Norfolk Safeguarding Adults Board (NSAB) commissioned a safeguarding adults review concerning the deaths of two adults at a private hospital Cawston Park.

During December 2020, the death of a third patient was added to the review’s remit. The deceased, Joanna, Jon and Ben were in their 30s. They had learning disabilities and had been patients at the Hospital for 11, 24 and 17 months respectively. They died between April 2018 and July 2020.

Dr Margaret Flynn, Chair of the National Mental Capacity Forum.

Dr Margaret Flynn is the current Chair of the National Mental Capacity Forum. Since 2019, Dr Flynn has been a Trustee of Anheddau Cyf, a not-for-profit charity supporting adults with learning disabilities, autism and mental health challenges across North Wales. 

Dr Flynn was also appointed as a Director of All Wales People First in 2018. She has been a Director of Flynn and Eley Associates Ltd since 2009 and has held various editorial roles for the Journal of Adult Protection since 1999. 

Dr Flynn has chaired and written several reviews concerning people with compromised mental capacity, in particular a review into the Winterbourne View Hospital during 2011. In 2013, she was commissioned by the First Minister of Wales to undertake a review of the neglect of older people living in care homes investigated as Operation Jasmine.

More recently, Dr Flynn chaired Wales’ inaugural National Independent Safeguarding Board and wrote the review concerning Cawston Park Hospital.

Joan Maughan, former Chair of the Norfolk Safeguarding Adults Board

Joan originally trained as a social worker (working for 24 years in social care with people with learning disabilities) where she became involved in the development of new approaches to service delivery for children and adults with learning disabilities.

She has been a Director for services to disabled children for the Children’s Society and a Regional Director for United Response, a voluntary sector provider for adults with learning disabilities focused on providing Supported Living and Day opportunities.

Joan was Chief Executive of the National Development Team for Inclusion (NDTI). Joan became the Chair of the Norfolk Safeguarding Adults Board in 2014 and in that role, was closely involved in the Cawston Park hospital safeguarding adult review. As a result, Joan advocated for the creation of a group to campaign at a local and national level and this led to the creation of the Coalition for Change.
